29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about November 17, 2026, and days prior at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 34, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. b) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 29, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. c) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 26, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. d) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 23, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. e) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 21, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. f) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 20, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. g) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 19, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. h) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the buffing wheel # 41, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. i) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the grinding belt # 15, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts. j) On or about November 17, 2026, at 16490 NW 13th Ave, North Miami, FL 33169; Finishing Department, employees operating the grinding belt # 17, were exposed to crushed-by and amputation hazards; in that, the employer did not ensure that the machine was guarded to prevent accidental contact with the point of operation, ingoing nip points, and rotating parts.
