29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: X-Test 7/8 Area: On or about August 29, 2025, and at times prior, employees and subcontracted employees were exposed to fall hazards without any form of fall protection when en route to work spaces. A subcontracted employee fell through an open HVAC shaft from an unprotected side or edge of the work platform and fell approximately 19 feet to the duct below.
