29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Worksite located at 612 Blees Industrial Drive, Macon, Missouri- Employees performing maintenance on shredding machines were exposed to fall hazards when they climb out of scissor lift and onto the top of the machines in which the employees are not protected from falling to the ground.

29 CFR 1910.303(b)(1):Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): Worksite located at 612 Blees Industrial Drive, Macon, Missouri- Employees were exposed to shock, electrocution and fire hazard posed by the condition of the electrical cords used to power the portable fans near the B2R machine. The cords had their outer insulation removed and the inner wires were exposed.
