29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about November 21st, 2025, employees engaged in maintenance operations located at 1228 South 16th Street, Omaha, Nebraska 68108, were exposed electrical, thermal, water, and air pressure hazards in that the employer failed to provide LOTO training for maintenance employees prior to them performing LOTO.   Specifically, the employer failed to provide LOTO training on washers #1-3 and Iron #1.

29 CFR  1910.147(c)(4)(ii):The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section.   On or about November 21st, 2025, employees engaged in maintenance operations located at 1228 South 16th Street, Omaha, Nebraska 68108, were exposed to variety of unidentified energy sources such as but not limited to  electrical, thermal, steam, and pneumatics.  The LOTO procedures for Iron # 1-3 did not clearly and specifically outline the following:   1.) The LOTO procedures did not identify the isolation points and how to specifically bleed residual pressure contained within the various systems.  2.) Verification on gauges was vague and did not contain clear instructions on how/which gauge to verify.   3.) The energy sources did not contain voltages, PSI, or temperatures.  4.) The LOTO procedures did not state a time for the irons to cool before work is performed.  5.) The LOTO procedures did not clearly state how to identify the various parts.  6.) The LOTO procedures states and lists a Manual Reference but does not state where the manual is or can be located if its needed to be referenced.  7.) The LOTO procedures do not contain a date of implementation and who authorized the procedures.  8.) The LOTO procedures do not state required tools or PPE.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 21st, 2025, employees engaged in production operations located at 1228 South 16th Street, Omaha, Nebraska 68108, were exposed to caught-in hazards in that iron  #2 had two rotating shafts on the right and left side of the iron that were not guarded from employees getting their hands caught in.
