29 CFR  1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times.  a) On or about November 13, 2025, the employer did not ensure  employees operating Crown PE4500-80 pallet jacks maintained a safe distance from each other in that an employee was injured when they pulled up within approximately 3ft of the pallet jack in front of them, tripped over the forks of the pallet jack in front of them, engaged the driving mechanism of their own pallet jack, and was subsequently pinned between the two pallet jacks, resulting in a broken left tibia and fibula.
