29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible and/or machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves:   (a) Shop floor; On or about November 19, 2025, and dates prior, the HOLZ?HER Edge Banding Machine (SN 191/1?708) had two protective hoods (doors) with interlocks that were bypassed/defeated, exposing employees to the hazardous moving parts inside the enclosure. The interlock tongue/key for each door had been unbolted from the doors and placed in the interlock holders, allowing the machine to operate with the hood doors open.  (b) Shop floor; On or about November 19, 2025, and dates prior, the Biesse Rover B 1650 CNC Machine (SN 1000009123) had an unguarded gap on the front left side of the perimeter fence that allowed employees to enter the rear of the machine without passing through the interlocked gate. This opening permitted direct access to hazardous moving parts, including the beam/trolley/carriage's traversing motion, while bypassing the intended safeguarding function of the interlocked enclosure.  (c) Shop floor; On or about November 19, 2025, and dates prior, the Busellato CNC Machine had two perimeter?fence sections removed, creating an 18?inch?wide gap at the front left side and a second gap approximately 4 feet wide at the front right side, that allowed employees to enter the rear of the machine without passing through the interlocked gate. Both openings permitted direct access to hazardous moving parts, including the beam/trolley/carriage's traversing motion, while bypassing the intended safeguarding function of the interlocked enclosure.
