29 CFR  1910.22(a)(3): Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a) On or about November 21, 2025, the employer did not ensure the ramp leading into the walk-in freezer was free of accumulated ice, exposing employees to slip and fall hazards.

29 CFR  1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a) On or about November 21, 2025, the employer failed to ensure exit routes were free and unobstructed in that empty cardboard boxes were staged in front of the doors, exposing employees to trip hazards in the event of an evacuation.
