29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: A) 23 Progress St. Edison NJ 08820-Blending room- Employees were exposed to struck by and fall hazards while an employee was being lifted by a Hyster Sit-Down electric powered forklift (Model#J35XNT; Serial#K160N07016R) utilizing an unapproved personnel lifting cage attachment (model# WP-36-18) with a capacity of 1000lb/450kg. The forklift truck's data and capacity plates were not changed to reflect the attachment. Condition noted on or about 11/21/2025

29 CFR 1910.178(q)(7): Powered industrial truck(s) which were used on a round-the-clock basis were not examined for defects after each shift: A) 23 Progress St. Edison NJ 08820-Blending room, employee(s) were exposed to injury when operating a Hyster Sit-Down electric powered forklift (Model#J35XNT; Serial#K160N07016R) with an attachment platform (model WP-36-18) designed for lifting personnel. The existing scheduled maintenance and inspection program was not tailored to include inspection for the lifting platform. Weld cracks and deteriorated harnesses were observed on the lifting platform. The program failed to address the need to examine load-bearing welds for cracks, which are critical for ensuring the platform's stability and safety. Condition(s) were noted on or about 11/21/2025.
