A) West end of overhead catwalk: On or about 11/21/25, the employer exposed employees to a fall hazard, in that a safe access was not provided to the work area beyond the guardrails of the catwalk. B) West end of overhead catwalk: On or about 11/21/25, the employer exposed employees to a fall hazard, in that the employees were not provided a safe means of access and egress, where employees climb onto a metal duct approximately 3' high and cross over the duct approximately 4' wide and climb/step down 3'8" onto a steel beam to access a work area.
