On or about November 21st, 2025 Location: 354 Harrison Ave, Harrison, NY 10528  a) Employees were exposed to approx. 20 ft fall  because the poles of pump jack scaffold were not secured at the bottom.

Employees did not have fall protection systems.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about November 21st, 2025 Location: 354 Harrison Ave, Harrison, NY 10528  a) Employees were exposed to approximately 26 ft fall while accessing a roof with a ladder that did not extend at least 3 ft above the top landing.
