This was observed on or about November 25, 2025, at 6100 County Road & Dove, Hebron, Nebraska. Permit required confined space danger signs were not posted in appropriate locations on the grain trailer.

The employer did not ensure that an attendant was present when an employee entered the grain trailer when grain was inside.

This was observed on or about November 25, 2025, at 6100 County Road & Dove, Hebron, Nebraska. The employer did not ensure that rescue procedures were developed to retrieve an employee inside the grain trailer. Employees went into the grain trailer to retrieve the employee without rescue equipment.
