29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a)Allied Plumbing and Heating Inc. at 615 East 4th Street, Pierre, SD 57501: On or about and at times prior to November 20, 2025, the employer did not ensure employees were adequately instructed to recognize and avoid hazards associated with the installation, testing, and removal of varying designs of testing equipment for pressurized piping systems. This condition exposed employees to stored energy and struck-by hazards.
