29 CFR  1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety:    a) On or before November 19, 2025, the employer did not ensure that employees who worked near potential electrical hazards were provided with training on electrically related safety practices. The employer allowed an unqualified employee to be exposed to electrical shock hazards as they were not trained and familiar with electrically related safety practices, prior to using a  Niuyhe 5-24 Foot Aluminum Telescopic Extension Pole to hang holiday lights on a tree while working from the ground within the minimum approach distance of uninsulated primary conductors that were energized with 7.2kV.  This exposed employees to electric shock hazards.

29 CFR  1910.333(c)(3)(i)(B): When an unqualified person is working on the ground in the vicinity of overhead lines, the person may not bring any conductive object closer to unguarded, energized overhead lines than the distances given in 29 CFR 1910.333(c)(3)(i)(A).  a) On or before November 19, 2025, the employer allowed an unqualified employee to be exposed to electrical shock hazards while using a conductive Niuyhe 5-24 Foot Aluminum Telescopic Extension Pole to install holiday lights on a tree that was adjacent to the uninsulated primary conductors that were energized to 7.2 kV. The employee made direct contact with the lowest power line with the pole which resulted in an electrical shock.

29 CFR  1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a) On or before November 19, 2025, the employer allowed an unqualified employee to be exposed to electrical shock hazards by using a conductive Niuyhe 5-24 Foot Aluminum Telescopic Extension Pole to hang holiday lights on an outdoor tree by primary uninsulated conductors that were energized with 7.2kV without any electrical protective equipment. The employee made direct contact with the lowest power line with the aluminum telescopic extension pole which resulted in an electrical shock.
