29 CFR 1904.29(b)(4): A form used to document employee injuries and illnesses was not equivalent to the OSHA 300, 301, and/or 300A:  Facility - The employer did not ensure that a spreadsheet and an incident report being used in lieu of an OHSA 301 was equivalent.  The documents were deficient, in the following instances:  Spreadsheet deficiencies: a) Completed by. b) Title of individual completing the form. c) Phone number for individual completing the form. d) Date entry was made. e) Complete address of employee that suffered the injury.  This would include city, state and zip code. f) The gender of the employee. g) Name of treating physician. h) Was the employee treated in an emergency room. i) Was the employee hospitalized. j) Case number on the 300 log. k) What was the employee doing prior to the incident.  l) If the incident resulted in an employee fatality, what was the date of the employee's death.  Incident report deficiencies: a) Phone number for individual completing the form. b) Complete address of employee that suffered the injury.   c) The location where treatment was administered away from the workplace. d) Was the employee treated in an emergency room. e) Was the employee hospitalized. f) Case number on the 300 log. g) If the incident resulted in an employee fatality, what was the date of the employee's death.
