29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.  On or about November 24, 2025, at an establishment located at 1518 South Broadway Street, Green Bay, Wisconsin, the Nissan Forklift (Model No.: KCPH01A15pv, Chassis No.: KCPH01P904635) was not removed from service due to tire chunking damage.

29 CFR  1910.243(b)(2): Airhose. Hose and hose connections used for conducting compressed air to utilization equipment shall be designed for the pressure and service to which they are subjected.  On or about February 25, 2026, at an establishment located at 1518 South Broadway Street, Green Bay, Wisconsin, the employer failed to ensure the hose and hose connections used for compressed air were designed for the pressure and service for which they were subjected.  Specifically, employees were utilizing an air nozzle that was connected to the air hose using a worm gear clamp rather than a crimped fitting.

29 CFR  1910.242(b): Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.  On or about February 25, 2026, at an establishment located at 1518 South Broadway Street, Green Bay, Wisconsin, employees using compressed air to clean debris off of equipment and their person using an air nozzle was not reduced to less than 30 p.s.i. and equipped with an effective chip guard.

29 CFR  1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about November 24, 2025, at an establishment located at 1518 South Broadway Street, Green Bay, Wisconsin, the employer did not verify that a workplace hazard assessment had been completed through a written certification when employees working in the Production Department were required to wear safety glasses, safety shoes, and thermal protective gloves.
