On or about February 25, 2026, at an establishment located at 1518 South Broadway Street, Green Bay, Wisconsin, the employer failed to ensure the hose and hose connections used for compressed air were designed for the pressure and service for which they were subjected. Specifically, employees were utilizing an air nozzle that was connected to the air hose using a worm gear clamp rather than a crimped fitting.
