In the loading area, southeast corner, located at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, the employer exposed employees to entrapment hazards in that, the area leading to designated exit doors was partially blocked by ventilation equipment and an attached hose.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a). In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, employees operating the NuCon Steel Model #620 Roll Forming Machines were exposed to amputation hazard due to lack of point of operation guarding.  b). In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, employees operating the NuCon Steel Model #735 Roll Forming Machines were exposed to amputation hazard due to lack of point of operation guarding.  c). In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, employees operating the NuCon Steel Model #755 Roll Forming Machines were exposed to amputation hazard due to lack of point of operation guarding.  d). In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, employees were exposed to a crush-by hazard when operating machines such as the Roto-Die Hydraulic Bender (Model-WC67K 100T, Serial No.210316) press brake, without a point of operation guard.

29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a). In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, employees were exposed to electrical hazards, in that, an electrical panel was blocked and did not have a minimum clearance of 36 inches.

29 CFR  1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a).  In the Metal Extrusion area at 1510 US Hwy 17 N, Wauchula, FL 33873: On or about November 25, 2025, the employer did not ensure that employees who were provided with and permitted to voluntarily use tight-fitting face piece such as 3M - 7500 Series Half Face Respirator with 6001 vapor cartridge and N95 filter pads, were provided with the information contained in Appendix D - Information for Employees Using Respirators When not Required Under Standard.  The Innovar Structures, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR  1910.134(c)(2)(i), which was contained in OSHA inspection number 1547203, citation number 2, item number 3 and was affirmed as a final order on 12-06-2021, with respect to a workplace located at 1510 US Hwy 17 N Wauchula, FL 33873
