1910.9(b) 29 CFR 1910.9(b): Training on hazards and related matters such as standards requiring employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, were not provided by the employer to each affected employee in the manner required by the standard. a) On 11/19/2025 at location 11021 Northcliffe Blvd, Spring Hill, FL 34608: Employer exposed employees to crush by hazards in that employees were not adequately trained in identifying the hazard of using a lift that was leaking the hydraulic fluid and the process for removing the equipment from service.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On 11/25/2025 at location 11021 Northcliffe Blvd, Spring Hill, FL 34608: Employer exposed employees to crush by hazards in that they continued to use lift number 8 for approximately 6 months after it was tagged out due to it started to leak hydraulic fluid and was unable to maintain it's height when lifting vehicles.
