Location: Worksite; rear of building.   a) On or about 11/25/25, employees were exposed to fall hazards of approximately 20ft to the ground below when crossing from one section of scaffold to another, on the same tier level. The scaffold does not have guardrails to protect employees from falling.

Location: Worksite; front of building.   a) On or about 11/25/25, employees using an extension ladder to access their work area on top of a flat roof were exposed to fall hazards of approximately 20ft from the ladder's side rails not extending at least 3 feet above the upper landing surface.
