29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On or about November 25, 2025, at 2914 33rd PL NW, Washington DC; Deck area, during the installation of the deck, employees were using rigid pneumatic nail guns without wearing appropriate eye protection, such as safety goggles or face shields. This oversight exposed them to a significant risk of eye injuries, including potential punctures, lacerations, or other trauma from flying debris and misfired nails.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.304(h)(1):Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  a) On or about November 25, 2025, at 2914 33rd PL NW, Washington DC. Ground level eastside; During the installation of the deck and soffit, the employees were operating a Dewalt table saw without the safety guard in place. The lack of the guard puts them at significant risk of serious injury, including potential amputation hazards, as the exposed blade could easily cause severe accidents.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records

29 CFR  1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On or about November 25, 2025, at 2914 33rd PL NW, Washington DC. Ground level eastside; The employees were using extension cords that were visibly damaged to power various equipment, including saws, which placed them at significant risk of electrical shock. The damaged sections of these cords revealed exposed internal wires, creating an unsafe working environment. Additionally, the power supply at the site was permanent and no ground faults circuit interrupters were used, further compounding the hazard as it connected directly to the compromised extension cords.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. Ground level eastside; the extension cords utilized to power equipment at the site, such as saws, were not equipped with ground fault interrupters (GFCI) while connected to the building's permanent electrical source. The condition left the employees exposed to potentially severe electrical shock hazards.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) On or about November 25, 2025, at 2914 33rd PL NW, Washington DC. Ground level eastside; the cords strain relief components utilized to power the electrical equipment involved in the operation of saws have been compromised. This failure has led to the internal wiring becoming exposed, creating a significant safety hazard. The damaged strained relief placed the employees at risk of electrical shock when these tools are plugged into the building's permanent electrical supply.
