29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) On the roof of the building - On or about November 25, 2025, employees were exposed to a struck-by hazard in that employees did not wear eye protective equipment while using pneumatic nail guns.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) On the West side of the roof- On or about November 25, 2025, the employer exposed employees to a fall hazard in that employees did not use fall protection while installing roofing shingles on a church building with a 6/12 slope roof and an approximately 11' fall to ground distance. Nava's Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.501(b)(13), which was contained in OSHA inspection number 1694320, citation number 1, item number 1 and was affirmed as a final order on December 19, 2023, with respect to a workplace located at 160 Minning Loop, Richmond Hill, GA 31324.
