On or about November 26, 2025, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about November 26, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately eight (8) feet seven (7) inches, above the ground, thereby exposing employees to fall hazards.  b.  On or about November 26, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately seventeen (17) feet six (6) inches, above the ground, thereby exposing employees to fall hazards.
