On or about November 18, 2025 Located at IR 670 Westbound 670 over Scioto River: The employer did not instruct their employee on how to use the company-built concrete cutting saw (serial number 20211155) correctly to recognize and avoid the unsafe practice when handling the equipment during work activities.

On or about November 18, 2025, the employer failed to ensure that ring buoys were provided and readily available for emergency rescue operations for employees performing concrete cutting near the north edge of a bridge passing over the Scioto River on 670 Westbound.
