Deficiencies noted included, but were not limited to, failing to use fall protection during masonry work, not having a safe way on/off a scaffold and using damaged planking on a scaffold. b) Job site, 341 Butler Street, Etna, PA: On or about December 18, 2025 during construction activities, the employer did not ensure a system of of hazard recognition and correction was implemented at the job site. Deficiencies noted included, but were not limited to, failing to use a safe way on/off the scaffold.
