29 CFR 1926.502(h)(1)(v):The safety monitor had other responsibilities which could took the monitor's attention from the monitoring function: a) Job site, 3431 Butler Street, Etna, PA: On or about November 24, 2025 during roofing work, the safety monitor was doing other tasks that took away from the monitor function. The monitor was moving a tool around, talking with other staff members and not watching at least one employee who was working at the edge of the roof.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) Job site, 341 Butler Street, Etna, PA: On or about November 24, 2025 during construction activities, the employer did not ensure employees working near wall openings were using appropriate fall protection systems/devices, exposing them to fall hazards of up to approximately 15 feet.
