29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.  On or about November 26, 2025, at the above addressed jobsite, employees working below roofing activities were not wearing hard hats, thereby exposing the employees to struck by hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about November 26, 2025, at the above addressed jobsite, employees performing roofing activities, eight (8) feet four (4) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about November 26, 2025, at the above addressed jobsite, employees performing roofing activities, eight (8) feet seven (7) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  c.  On or about November 26, 2025, at the above addressed jobsite, employees performing roofing activities, seventeen (17) feet seven (7) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about November 26, 2025, at the above addressed jobsite, employees performing roofing activities had been provided fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
