29 CFR 1926.452(w)(6)(i):Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions: a) Billings Limited Army Aviation Support Facility, 3648 AJ Way Billings, MT: On or about November 21, 2025 and at times prior employees were exposed to fall hazards while welding hanger door track from the Snorkel S2770RT scissor lift at a height of approximately 27 feet in close proximity to unprotected floor holes.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Billings Limited Army Aviation Support Facility, 3648 AJ Way Billings, MT: On or about November 21, 2025, the employer failed to report an employee hospitalization as a result of a work-related incident, within (24) twenty-four hours. An employee was hospitalized after a job site fall on November 21, 2025 and OSHA was notified on November 24, 2025.
