29 CFR 1910.109(g)(2)(v): All electrical switches, controls, motors, and lights located in the mixing room did not conform to the requirements in subpart S of this part for Class II, Division 2 locations. At a worksite located at or near 2429 East 535th Road, Buffalo, MO 65622: Employees were exposed to fire and/or explosion hazards. This was demonstrated on or about November 26, 2025, and at times prior to when equipment including but not limited to Central Machinery Cement Mixers were used in the manufacturing of stars and was powered by a plug and cord that was run through an opening in the wall. The employer did not ensure that all electrical switches, controls, motors, and lights located in the mixing room met the requirements in subpart S of this part for Class II, Division 2 locations.

29 CFR 1910.109(g)(2)(vi)(c): The entire mixing and packaging plant was not cleaned regularly and thoroughly as to prevent excessive accumulation of dust. At a worksite located at or near 2429 East 535th Road, Buffalo, MO 65622: Employees were exposed to fire and/or explosion hazards. This was demonstrated on or about November 26, 2025, and at times prior to when comp dust was allowed to accumulate due to inconsistent and poor housekeeping in manufacturing areas of the facility, including but not limited to mixing and packaging areas. Comp dust or composition dust is comprised of black powder and metal salts which are combustible and can catch fire under the right conditions. The employer did not ensure that the entire mixing and packaging areas of the facility/plant were cleaned in a regular and through manner as to prevent the accumulation of dust associated with the manufacturing of fireworks.
