Second Floor Men's Shower Room: On or about December 2, 2025, and at times prior to, the Belkin 11-Outlet Relocatable power taps (power strips) were series-connected (daisy-chained) to extension cords or other power strips. These conditions potentially exposed employees to electrical and fire hazards.

29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. 2nd Floor LAN Room: On or about December 2, 2025 and at times prior to, a commercial Sentry ABC Dry Chemical fire extinguisher was obstructed by a fan and computer monitor. This condition potentially exposed employees to fire and respiratory hazards.
