Jobsite: On or about October 23, 2025, the employer did not ensure that the employee replacing residential siding approximately 26 feet from ground level was protected from fall hazards by the use of personal fall arrest systems or guardrail systems.

29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds.  Jobsite: On or about October 23, 2025, the employer did not ensure that a competent person was designated to oversee the safe erection and use of the ladder-jack scaffold used by an employee to replace siding on a residential home. The scaffold platform was approximately 26 feet from ground level and no fall protection was given to the employee working from the scaffold.

Jobsite: On or about  October 23, 2025, the employer did not ensure that the working platform on a ladder-jack scaffold did not exceed 20 feet in height. An employee worked on a ladder-jack scaffold that measured approximately 26 feet from ground level.

Jobsite: On or about October 23, 2025, the employer did not ensure that an employee replacing siding on a residential home from a ladder-jack scaffold platform approximately 26 feet from ground level was trained to recognize the hazards of working without fall protection.

Jobsite: On or about October 23, 2025, the employer did not ensure that an extension ladder used to access the working platform of a ladder-jack scaffold reached no less than two feet from the scaffold platform.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Jobsite: On or about October 23, 2025, the employer did not ensure that employees required to use ladders to install siding on a residential home were trained in the safe use of ladders prior to performing the work. This exposed the employees to fall hazards.
