29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds.  Jobsite: On or about October 23, 2025, the employer did not ensure that a competent person was designated to oversee the safe erection and use of the ladder-jack scaffold used by an employee to replace siding on a residential home. The scaffold platform was approximately 26 feet from ground level and no fall protection was given to the employee working from the scaffold.

Jobsite: On or about October 23, 2025, the employer did not ensure that an extension ladder used to access the working platform of a ladder-jack scaffold reached no less than two feet from the scaffold platform.

Jobsite: On or about October 23, 2025, the employer did not ensure that employees required to use ladders to install siding on a residential home were trained in the safe use of ladders prior to performing the work. This exposed the employees to fall hazards.
