29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection are protected from being cut, abraded, melted, or otherwise damaged. Site- On or about November 25, 2025, an employee felling a tree fell approximately 30 feet when they cut through their positioning device with their chain saw. The employee was using a single steel core lanyard as a positioning device for fall protection; the employee was not using a second means of fall protection at the time of the fall.
