29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a) On November 29, 2025, at Tractor Supply Company, located at 1101 Brookside Lane in Bismarck, North Dakota, the employer did not ensure forklift operators was moving load that was safely secure or stable on the forks while loading a gun safe standing upright into the bed of a pickup truck. This condition exposed an employee to struck-by and crushing hazards involving the load tipping off the forks.
