29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket): At the workplace located at 1126 Adams Street, Kansas City, Kansas, employees were exposed to electrical hazards in the following instances: (a) Employees using a black charging cord for plugging in pallet jacks at the west wall were exposed to electrical hazards due to the plug end of the charging cord exposing the energized internal conductors. (b) Employees using a black charging cord for plugging in pallet jack #22 at the west wall were exposed to electrical hazards due to the outer jacket insulation being damaged, thus exposing the energized internal conductors.
