At a temporary worksite, located at the east side of 12051 Tribute Circle lot 48 Jacksonville, FL: On or about December 3, 2025, the employer exposed employees to struck-by hazards, in that protective eyewear was not utilized by employees while using a metal hammer and pneumatic nails guns for framing work.

West side: At a temporary worksite of 12051 Tribute Circle lot 48 Jacksonville, FL: On or about December 3, 2025, the employer was exposing an employee to a 18- 19 foot fall hazard, in that the employee was not tied off when conducting framing work.
