OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) On the roof of the site, located at 19100 S Tamiami Trail, Fort Myers, FL, 33908, on or about 10/15/2025, the employer exposed employees to struck-by hazards, in that the employer did not ensure that the 5 inch diameter 7/8th inch arbor abrasive wheels used on the 4.5 inch diameter 5/8th inch arbor DeWalt DWE4120 Angle Grinders were the proper size for the grinders.
