29 CFR 1910.147(a)(3)(i): The employer did not establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees: a) Worksite, Bailing Machine: On or about 12/3/2025, authorized employees are required to utilize lockout/tagout and the employer did not establish a program and procedures for the control of hazardous energy. Employees are exposed to the hazard of unexpected energization, startup, or release of stored energy in machinery.
