29 CFR 1910.147(d)(6):Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: a) Phase 3; On or about November 5, 2025 and times prior the employer failed to ensure employees working at or near the Phase 3 Dry Product (DP) lines below Cyclone A verified the isolation and deenergization of machines and equipment which had been locked out before starting work.
