29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Storage Site Gershow Recycling Corporation, 71 Peconic Avenue, Medford, NY, 11763 - An employee on site was exposed to fall hazards while trying to untangle a wire rope on a Liebherr LTM 1160 cranes hoist winch. The employee did not have any means of fall protection and fell off the crane approximately 12'8" to the ground resulting in injury; on or about December 3, 2025. The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employee was not trained to recognize the hazards associated with falls; On or about December 3, 2025. The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
