29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: Site: 700 Bartley-Chester Road, Flanders, NJ 07836. 1). On or about December 4, 2025, G&S Operations LLC did not install a warning line system 6 feet or more away from the roof edge exposing the employees who were engaged in solar panels installation to a fall hazards of approximately 30 feet from the ground below.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: Site: 700 Bartley-Chester road, Flanders, NJ 07836. 1). On or about December 4, 2025, G&S Operations LLC failed to ensure it's employee who was assigned as the monitor for a warning line system during a solar panel installation on roof had no other responsibilities other than monitoring the system. This hazard exposed employees to possible falls of approximately 30 feet from the ground below.
