On or about December 4, 2025, G&S Operations LLC failed to ensure it's employee who was assigned as the monitor for a warning line system during a solar panel installation on roof had no other responsibilities other than monitoring the system. This hazard exposed employees to possible falls of approximately 30 feet from the ground below.
