In the work platform (man-basket) of an aerial lift, at a temporary worksite located at a commercial business, 3434 Atlantic Boulevard, in Jacksonville, FL: On or about December 4, 2025, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while operating an aerial lift performing stucco work.
