On or about December 4 2025, at a work location located at 3130 Dover Rd, Silver Lake Ohio 44224; the employer did not ensure eye protection was utilized by employees using pneumatic nail guns and hand tools to install roofing shingles, exposing the employees to struck-by hazards to the eyes.

Employees were exposed to falls of 8.9 feet above the ground while tearing off and installing roofing shingles on a roof without utilizing any form of fall protection.
