29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about December 4, 2025; Eastside of the building;  employees were not wearing head protection while working under or near the dismantling of the scaffold system.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(f)(7):Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) On or about December 4, 2025; Eastside of the building; The employer had not ensured that a competent person who understood the hazards associated with the dismantling of the scaffold such as instability and how to abate those hazards which include the use of fall protection understood his role during the process. The lack of a competent person exposed the employees to a fall hazard of 24-30 feet while dismantling the scaffold.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) On or about December 4, 2025, on the east side of the building, the employer had failed to ensure that while dismantling the scaffold system, the employees were protected from falling an estimated 24-30 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.454(b):The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  a) On or about December 4, 2025; Eastside of the building; The employer had not ensured that a qualified and experienced individual adequately trained employees to recognize the potential hazards associated with dismantling scaffolds. This training was critical for informing employees about risks such as falling objects, improper equipment handling, and the instability of scaffolding structures during disassembly. Furthermore, the employer failed to provide comprehensive instruction on effective methods to abate these hazards, including the proper use of personal fall-protection systems to safeguard workers throughout the dismantling process.
