29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE. a) On or about December 3, 2025, at a worksite located at 2702 17th Street NE, Washinton, DC, Roof area, employees of Solar Solution DC were exposed to an estimated 30-40-foot fall hazard while engaged in solar installation activities on a residential roof without fall protection. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about December 3, 2025, at a worksite located at 2702 17th Street NE, Washinton, DC, rear of site, employees of Solar Solution DC were exposed to a fall hazard while accessing the roof or ladder. The aluminum extension ladder was placed an estimated 12 inches above the landing area (roof). The position increases the ladder from shifting, tipping or displacement during transition to the roof or ladder. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(8):Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. a) On or about December 3, 2025, at a worksite located at 2702 17th Street NE, Washinton, DC, rear of site, employees of Solar Solution DC were exposed to a fall hazard while accessing the roof or ladder. The aluminum extension ladder was placed an estimated 12 inches above the landing area (roof) and not secured. The ladder not being secured increases the ladder from shifting, tipping or displacement during transition to the roof or ladder.
