29 CFR  1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports:  a) On December 5, 2025, employees working from tubular welded framed scaffold platform were exposed to falling  approximately 10 feet where the scaffold platform was not fully planked.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On December 5, 2025, employees engaged in framing carpentry work to a newly built residential structure were not protected from fall hazards while working at elevations approximately 13.8 feet above the ground.
