29 CFR 1926.453(b)(2)(v):An employee was engaged in facia activities and wore a personal fall arrest harness but a lanyard was not attached to the boom or basket when working from an aerial lift exposing the employee to fall hazards of approximately 40 feet to the sidewalk below. a- On 12/03/2025 CSHO observed that an employee engaged in facia activities from the aerial lift basket was exposed to fall hazards of approximately 40ft to the sidewalk below as a personal fall arrest was donned but not secured via a lanyard to the aerial lift basket. Condition noted on or about 12/03/2025.
