At a new multi-family construction site located at 8600 N Main, KC MO -- employees engaged in siding work were using pneumatic nail guns without eye protection. This exposed the employees to getting shrapnel and/or debris in their eyes from the pneumatic nail gun causing injury to their eyes. This was documented on or about 12/4/2025.

This exposed the employee to falling out of the basket to ground level as he was not wearing any fall protection. This was documented on or about 12/4/2025.
