29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  At a multi-family worksite located at 8600 N Main KC MO-- employee engaged in framing duties was working from a job-made scaffold that was not fully planked/decked. This exposed the employee to fall hazard of falling to balcony below or the ground. This was documented on or about 12/4/2025.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms.  At a multi-family worksite located at 8600 N Main Bldg B KC MO -- employee engaged in framing duties while working from job-made scaffold was not protected with guardrails of any kind. This exposed the employee to fall hazards less than 10 feet to the balcony below. This was documented on or about 12/4/2025.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a new multi-family worksite located at 8600 N Main St, KC MO -- employees engaged in framing duties while working from an unguarded balcony and not using/wearing any personal fall arrest system was exposed to fall hazards greater than 10 feet above ground level. This was documented on or about 12/4/2025.  Gonzales Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1666635, citation number 1, item number 2 and was affirmed as a final order on 9/6/2023, with respect to a workplace located at Lots 31 & 32 Aurora St, Olathe KS.  Gonzales Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1739545, citation number 2, item number 1 and was affirmed as a final order on 9/20/2024, with respect to a workplace located at 22108 W 123rd Pl, Olathe KS.
