1910.134(d)(1)(iii) The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  Procter & Gamble Manufacturing Company located at 3875 Reservoir Road in Lima, Ohio:  On or about November 12, 2025, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an oxygen deficient atmosphere while entering the lower valve garden (LVG) of finished product reblend tanks 624, 625, and 626 in the HDL tank farm area. Employees were not protected against hazards associated with the lack of oxygen created by the introduction of nitrogen from the reblend tanks into the LVG for reasons such as, but not limited to, collecting final product samples.
