29 CFR  1910.132(b): Where employees provide their own protective equipment, the employer did not assure its adequacy, including proper maintenance, and sanitation of such equipment.  Great Lakes Welding, LLC - Oberlin, Ohio: On or about December 2, 2025, the employer did not ensure that employee-provided clothing and eye protection, used to protect against burn injuries and light radiation, was adequate for employees performing MIG welding. Cotton clothing for the upper body did not provide adequate coverage and an employee-provided welding helmet was cracked and not in good repair.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment.   Great Lakes Welding, LLC - Oberlin, Ohio: On or about December 2, 2025, the employer's hazard assessment did not protect employees who performed metal inert gas (MIG) welding from burn and fire hazards.  The employer did not select and have each affected employee use personal protective equipment that would protect against burn injuries in that synthetic clothing was worn and a welding helmet with an inadequate shade lens was utilized. An employee performing MIG welding sustained fatal burn injuries when their clothing caught fire.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.132(f)(1)(v): Employee(s) required to use PPE by this section were not trained to know the proper care, maintenance, useful life, and disposal of the PPE.   Great Lakes Welding, LLC - Oberlin, Ohio: On or about December 2, 2025, the employer did not ensure that employees who utilized flame resistant clothing to protect against burn injury during welding operations, were trained in the care, maintenance, and useful life of the personal protective equipment. Employees who performed welding operations with a 100% cotton shirt were exposed to burns and fire hazards in that the protective clothing did not provide adequate coverage.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation.  Great Lakes Welding, LLC - Oberlin, Ohio: On or about December 2, 2025, an employee who wore a welding helmet with a Shade No. 9 lens was exposed to injury from radiant energy in that the shade was inadequate for the metal inert gas (MIG) welding tasks performed.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
