29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a) On or about 12/4/2025 on the site located at 1814 Highway 131, Massena, NY: An employee returning a piece of bath from the pot room deck into pot 147 slipped on the sill plate causing his left calf to enter the molten metal bath. The employee sustained 2nd/3rd degree burns to the left calf.

29 CFR 1910.28(b)(11)(ii):Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: a) On or about 12/4/2025 on the site located at 1814 Highway 131, Massena, NY: Employees were exposed to fall hazards when using stairs not equipped with handrails to access the pots. Stairs incorporated a step onto the deck as part of the set which gave each level change 3 treads and 4 risers.
