29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi: (a) On or about December 8, 2025, and at times prior, the employer did not inspect hose connections, fasteners and clamps on acetylene cylinders and their supply lines, to ensure that the materials and methods used are appropriate for the type, size, and pressure rating of the hose. A set of oxy/acetylene hoses had been connected with water hose clamps. This condition exposed employees to fire and explosion hazards.
